About the Assignment
The case manager is responsible for coordinating, monitoring, and facilitating services for the residents in the Elijah Rising Restorative Care Program. Duties include case management, transportation, forming relationships with residents, and documenting resident groups, activities, and appointments on a daily basis. This role includes meeting with residents weekly to set goals for ongoing development and program progress, following up and providing tools and assistance toward those goals, and ensuring forward movement in the residential care plans. This can include aid with medical needs, public assistance, educational goals, completion of program work, personal improvement goals, and more. The residential life coordinator plays a key role in the residents’ lives as they are a constant, stable presence and therefore must be a role model and a good listener. Good communication skills are key.
Required duties and responsibilities:

Essential Functions
Assists with the daily needs of the residents, including helping to identify those needs and advising Residential Life Manager
Daily facilitates and/or oversee group activities and curriculum work
Transports residents to appointments and takes appropriate notes for documentation
Executes all case management for residents
Aids in conflict resolution and facilitating healthy communication between residents
Daily enters all documentation and notes into client management software to ensure accurate and timely reporting

Position Type / Expected Hours of Work
This is a full-time position with an expectation of 40 hours per week.

Some travel is expected for this position, driving up to 2 hours for some appointments.

Education and Experience
Bachelor’s Degree preferred
One to two years of social/victim services experiences, preferably trauma-informed
Working knowledge of social work practices
Good computer skills and the ability to learn client management software quickly
Must have excellent people skills with the goal of developing trust with staff and residents in order to assist them with effective care.
Must be detail-oriented in order to effectively track and report metrics.
Must be able to balance programming with administration to meet all deadlines.
Must have knowledge of industry methods and best practices.
Computer and Communication Skills
Excellent computer skills needed.
Able to create spreadsheets, graphs, charts, presentations and other documents.
Must be able to manage time efficiently to meet expected weekly deadlines.
Able to read, analyze, and interpret professional journals and general business periodicals.
Able to write reports, presentations, blogs, essays, as needed.
Able to speak publicly and present to different groups as needed.
Mathematical and Reasoning Skills
Must be able to work with mathematical concepts such as fractions, percentages, ratios, and proportions to practical situations.
Able to define problems, collect data, establish facts, and draw valid conclusions.
Able to interpret an extensive variety of technical instructions.

Must be able to walk, see, and hear. Employee is frequently required to stand, walk, reach, and drive using legs, arms, and hands. Vision requirements include close vision, far-range vision, and the ability to focus. Hearing requirements include the ability to hear normal conversation in normal ranges and the ability to be in moderate to loud environments for extended periods of time.
Other physical requirements specific to this job:
Occasionally required to lift/carry and push/pull up to 50 lbs.
Occasionally required to climb stairs, bend or stoop, kneel or crouch.
Other Requirements

Must Have
Must have reliable transportation.
Must possess a valid state driver’s license and valid vehicle insurance.
Must be available to oversee and ensure staff coverage for 24/7 care as needed during emergency situations.
Must be able to drive a 15-passenger van.
Must be able to drive on the highway for long distances (2 hours+).
Must submit to and pass a pre-employment drug test and periodic drug tests during employment.
Must be able to pass all background screens.

Education, Training, and Experience
  • High School Diploma
  • One to two years of case management experience, preferably trauma-informed
  • Knowledge of general social work practice and specific knowledge of trauma
  • Must complete Elijah Rising’s Basic Training and Restorative Care Orientation
