Coronavirus Special Agreement

As we respond to this opportunity to serve our partners, here are best practices that we are asking everyone to follow for safety as well as to reduce the spread of the virus.

Please arrange to volunteer at another time if:
• You have had contact with a person who has tested positive for COVID-19 or have been exposed to someone quarantining because of a presumed positive test result or who personally believes to be infected because of symptoms: fever, cough, sore throat, loss of appetite, fever, fatigue, loss of taste or smell
• You have had a fever in the last 14 days or been exposed to anyone with a fever.
• You have recently traveled internationally or to/from a high-risk location.

General Guidelines:
• Volunteers age 15-17 must be accompanied by a parent. No one under the age of 15 will be allowed to volunteer at this time.
• Only those who have registered will be able to volunteer. Please make an account for all family members and register appropriately.
• Please note that we cannot sign off on court-ordered community service hours without prior notification. If you would like to volunteer for court-ordered community service, please email
• If possible, maintain a distance of at least 6 feet.
• Practice proper hand hygiene. Wash your hands with soap and water for at least 20 seconds. If soap and water are not readily available, use an alcohol-based hand sanitizer with at least 60% alcohol.
• Do not touch your face with unwashed hands. Cover your nose and mouth with a tissue when coughing or sneezing; throw used tissues in the trash. If a tissue isn’t available, cough or sneeze into your elbow or sleeve, not your hands.
• After you volunteer, wash all clothing daily (including jackets) and practice proper personal hygiene daily.

2021 Volunteer Agreement and Release
I, the undersigned, wish to volunteer my services to various community service organizations and projects through you, Christian Service Mission, an Alabama nonprofit corporation (“Christian Service Mission”). In consideration of your locating, arranging, coordinating and/or making available volunteer opportunities. I hereby agree and release you as follows:
I acknowledge and agree that the nature of the volunteer services which are typically performed by Christian Service Mission volunteers, and which may be performed by me as a Christian Service Mission volunteer, may involve (a) physical activity (including without limitation work with heavy tools and materials), (b) contact with unidentified and unfamiliar persons, (c) travel to and from various unspecified locations, and (d) other potential risk of injury. Notwithstanding the preceding sentence, I willingly and freely agree to volunteer and hereby assume any and all risk, with respect to any liability of Christian Service Mission for such risk, including without limitation risk of any accident or injury to person or property which I may sustain in connection with my participation as a Christian Service Mission volunteer or in any Christian Service Mission related project or activity.
The undersigned hereby releases you and your directors, officers, partners, agents, employees, successors, assigns, licensees, sponsors, donors, volunteers, representatives, guest and affiliates (collectively, the “Christian Service Mission Related Parties”) from and covenants not to sue you or any Christian Service Mission Related Party for, any and all claims and causes of action, whether known or unknown, arising out of, based upon or relating to my participation as a Christian Service Mission volunteer or in any Christian Service Mission activity or project. I hereby acknowledge that Christian Service Mission is an organization involved in assisting organizations and associations which may from time to time be involved with child care and related matters. I hereby confirm, represent and warrant that I have never been convicted of a violent crime, child abuse or neglect, child pornography, child abduction, kidnapping, rape or sexual offense, nor have I ever been ordered by a court to receive psychiatric or psychological treatment in connection herewith.
I further irrevocably grant to Christian Service Mission, its assigns and successors, my consent and full right to use my name, photograph, likeness, image, voice and biography in any and all media, publications, advertising, and publicity, in connection with my participation hereunder.
This release shall inure to your benefit, as well as to the benefit of your successors, licensee, agents, employees, affiliates and assigns. This release shall be governed by the laws of the State of Alabama.
